Patient Care and Clinical Services:
Provide comprehensive primary care services including health assessments, diagnosis, treatment, and management of acute and chronic medical conditions across the lifespan.
Deliver integrated behavioral health services, including screening, diagnosis, and treatment of common mental health conditions such as depression, anxiety, substance use disorders, and other psychiatric illnesses.
Develop and implement individualized care plans that address both physical and behavioral health needs, ensuring holistic patient care.
Perform routine wellness exams, preventive screenings, immunizations, and health promotion education.
Prescribe medications, including psychotropic medications, and manage medication adherence and side effects.
Behavioral Health Integration:
Collaborate with interdisciplinary teams including psychiatrists, psychologists, social workers, and other healthcare providers to coordinate patient care.
Conduct brief interventions, motivational interviewing, and crisis management for patients experiencing behavioral health issues.
Provide psychoeducation to patients and families on mental health conditions, treatment options, and coping strategies.
Utilize evidence-based behavioral health interventions such as cognitive-behavioral therapy (CBT) and mindfulness-based stress reduction.
Patient Education and Advocacy:
Educate patients on self-management of chronic illnesses, lifestyle modifications, and behavioral health management.
Advocate for patients to ensure access to necessary medical and behavioral health services, including referrals to specialists and community resources.
Empower patients through education and support to make informed decisions about their healthcare.
Quality Improvement and Compliance:
Participate in quality improvement initiatives aimed at enhancing the integration of behavioral health into primary care.
Ensure compliance with all federal, state, and local regulatory requirements, including HIPAA and other privacy standards.
Maintain accurate and timely documentation in electronic health records (EHR) for all patient encounters, assessments, and treatment plans.
Professional Development and Leadership:
Stay current with advancements in primary care and behavioral health through continuing education, professional development, and evidence-based practice.
Provide mentorship and training to nursing staff, students, and other healthcare professionals in integrated care practices.
Participate in community outreach programs to promote mental health awareness and access to integrated care services.
Administrative Duties:
Manage patient caseload efficiently while ensuring high standards of care.
Collaborate with administrative staff for scheduling, billing, and other operational needs.
Participate in program development, policy formulation, and strategic planning for integrated care services.

Temah Healthcare Services, LLC started as a house call company in 2013 to meet the healthcare needs of the underserved and homebound patients. In 2015, we expanded into family practice, and in 2018, we started integrated services to include medical and behavioral Healthcare Services.

We are accredited by CARF International and offer state licensed programs including Outpatient Mental Health Clinic (OMHC), Psychiatric Rehabilitation Program (PRP), and Mobile Treatment (MTS).
We provide services, both in person or via telehealth, aiming to serve individuals from ages 3 and older suffering from mental and behavioral disorders and individuals from ages 0 and older requiring medical care with the intent of attaining health, wellness, recovery, and wholeness using a holistic approach.
